# Application for changing contact information

This is an example form for changing a ZoneID account’s email address

> In order to restore access to services in the Zone Media LLC control panel please replace my current contact email address with a new one.
>
> SERVICE: (server and/or domain)
> NEW EMAIL ADDRESS: (the email address to which the new password for the ZoneID will be ordered)
> NAME AND SURNAME:
> COMPANY / ORGANIZATION AND REGISTRATION CODE: (NB! If the owner of services is a legal entity, we ONLY accept applications signed by a board member / legal representative of the company).
